Clinical Placement D6

Day 6 : 24 th May 2016 I arrived at icu around 6.50 am. Surprisingly, there's no patient at all. Pheww! One of the staff nurses once said that nil patient would be on once in a blue moon. So I guess, today is the day. So there's not much we're doing today. We did the dusting, bed making, organised this and that, top-up requisites and so on and so forth, The sister told us on how the ECG machine works. The speed, the filter, the gain and its setting. Why we should not plug in the plug when we're doing the ECG on the patient? It is because the electrical current on the leads and the body will be interrupted by the electrical current from the plug. If the readings are artificial, what should we do? Repeat ECG Check if there's any faulty on the system or any wiring. Avoid any interruption such as no mobile phones around, switch off any socket and so on. What kind of lung disease that should not be given oxygen therapy? ...

Clinical Placement D5

Day 5 : 23 rd May 2016 Today's nothing much that I wanted to share here. We're just being explained about cardiac pacing. There's internal acute pacing that can be inserted in the hospital itself as of temporary insertion only. But if it is chronic, then the permanent pacemaker will be inserted. So there's important steps to be revised and apply in the procedure. You need to be fully understood then you are able and capable of doing the procedure. Atrial Fibrilation (AF) is an incomplete induction of the heart. Patient would have incomplete rhythm of the heart. As P will be not 'appeared' in the ECG rhythm. The patient will have shortness of breath (SOB). Okay, that's all for today. Love,         D.

Clinical Placement D4

Day 4 : 19 th May 2016 My colleague (Fatin) and I listened and involved in the passing report. So we only had one patient which was admitted in the afternoon of 18 th May 2016. This patient has Supra Ventricular Tachycardia (SVT). The SVT is an abnormally fast heart rate of more than 60 beats per minute. This patient has around 115 beats per minute. So during the doctors' round, the patient has questions and answers session with the doctors. The doctors explained to him what should be done to reduce the heart rate per minute. The doctor gave opinion of first, taking medicine and see the outcomes and if the medicine do not show any positive outcomes, operation would be the ones. Then the patient asked where should the surgery be done. So the doctor said most patients were going to Jerudong Park Medical Centre (JPMC) or to Singapore. The doctor informed the patient that it would cost around BND 40K per surgery. It really shocking tho, the price. Huhu... Around ...

Clinical Placement D3

Day 3 : 18 th  May 2016 My colleagues and I were managed to listen to the passing report from night duty staff nurses. So there are only 3 patients in total. One patient to transfer out to the medical ward. Whereby one patient to have a CT Scan (Computerized Tomography Scan) to RIPASH as the CT scan machine in SSBH is still under maintenance. So we have to prepare the transport system (ambulance), the sending off forms, the items that are required in case of emergency such as the Automated External Defibrillators (AED), the oxygen tank. The patient is ready sitting on the wheelchair. Then we went to the ground floor as the ambulance was awaiting us. After sending off the patient, we went back to the unit and we had a long briefing on the drugs names and its indication with the Sister. He surely has a lot of knowledge on the medications'   indication. We surely learned a lot. The drugs that we discussed are mostly and commonly used in the ICU. Oh yeah, the Sister s...

Clinical Placement D2

Day 2 : 17 th May 2016 Today is the second day of clinical placement. So, we got to join and listen the passing report. Then we joined with the doctors' round. I got the opportunity to do the pus swab sample on the CVP line as the area were moist and a bit wet. So the pus swab is to indicate if there is a risk of infection occurred on the site. Then the staff nurse remove the CVP line as instructed by the doctor. Sterile technique is required when removing the line. Then I got to know a little bit about this one patient. She are willingly to share with me regarding her condition. She has renal problem that she has to do the Haemodialysis. After that we had some discussion that are very useful for our learning, especially for our exam (as emphasized by the Sister). The discussion are more related to AMSHS and Life Science. We discussed about how to handle patient with Asthmatic disease/Breathing problem, patient with Acute Coronary Syndrome (Chest pain), Appendectom...

Clinical Placement

Day 1 : 16 th May 2016 Today is the first day of our 7 th rotation of Clinical Placement. So this rotation, I was assigned in the Intensive Care Unit (ICU) in Suri Seri Begawan Hospital (SSBH) in Belait District. There are four of us assigned in this unit. I was a bit nervous because this is the first time we were located in different hospitals among our colleagues. Some at Tutong Hospital, Renal Centre in Kiaraong and Rimba, and others are at RIPAS Hospital. The staffs are very welcoming us in the unit and do a short briefing/orientation around the unit itself. Then we were called by the Matron and Sisters to meet them for the first time. They gave us briefing and a short welcoming speech. Then we had a 'hospital-round' to familiarize with the hospital setting or layout. We even went to the Medical and Surgical wards as well as the Obstretic and Gynaecology ward. Briefing from the Sister regarding the ICU settings. The roles of the staffs in the ICU T...
